Graft count still matters — just not to the bill. It decides how long the day takes, whether the case is better split across two sittings, and how much donor supply is left for a second procedure years later. Those are surgical decisions made at assessment, not numbers you can settle from a photograph.

A flat package price tells you the total before you book a flight. A per-graft price tells you the total after you have landed. That single difference is where most billing disputes in this sector begin, and it is worth settling before you compare any two quotes.
Per-graft pricing is not dishonest in itself. It is a defensible way to charge, and a clinic that estimates carefully lands close to its own quote. The difficulty is structural rather than moral: the person deciding how many grafts you need is also the person paid by the graft, and the decision is made at the point where walking away costs you a flight, a hotel and the time you booked off work.

Because they are not selling the same operation. Two Istanbul clinics can quote figures that are far apart while describing the procedure in near-identical words, and almost all of the gap is explained by two things: how many patients the clinic runs through in a day, and who performs which stage of the surgery.

A hair transplant is not one task. It is extraction, channel opening, and placement. Extraction and placement can be carried out by trained technicians under supervision, and in most clinics they are. Opening the recipient channels — the angle each hair leaves the scalp at, the depth, the direction, the density — is the stage that decides whether a hairline reads as natural a year later. It is also the stage most quietly delegated, because it is the one a patient cannot see happening and cannot audit afterwards.
That is the fork in the road. A clinic built for volume needs the surgeon’s hours spread across as many patients as possible, so the surgeon marks the hairline, greets the patient and moves on. A clinic built around one surgeon operating on a small number of patients a day cannot spread that cost, so the figure is higher. Neither is a scandal. Both are business models — and the price is telling you which one you are buying.

So the useful question is not “why is that clinic cheaper” but “what is the cheaper clinic doing differently, and does it matter for my case”. Ask both clinics the same three things: who opens the channels, how many patients the surgeon operates on that day, and whether the quoted figure can change after arrival. The answers separate the two models faster than any price list does.

With a flat all-inclusive package the surgical and in-Istanbul costs are already inside the figure: the procedure, private transfers, hotel nights around the operation date, the medication set, the first wash at the clinic and follow-up afterwards. What is left is what you arrange yourself — and it is worth pricing before you compare any two clinics.
We do not publish figures for any of these, because they are not ours to quote — a flight from Manchester in February and a flight from Toronto in July are not the same purchase, and a page that averages them is guessing. Price them for your own trip, add them to the package figure, and compare that total against a quote from home.
Compare totals rather than headlines, and make sure both totals cover the same things. A home-country quote usually covers the operation and little else; a Turkish package usually covers the operation plus the logistics around it; and neither includes your flights unless it says so in writing.

The saving is real and it is structural: operating costs are lower, the local market is competitive, and experienced teams stay busy enough to keep prices down. What the saving does not do is answer any of the six questions above for you. A cheap operation performed by whoever was free that morning is not a saving, and a second procedure to correct a first one removes the discount entirely.
Eyebrow and beard work is quoted case by case rather than from the scalp tiers, because the graft numbers and the placement demands are different. Both are more exacting per graft than a scalp case: the angle at which each hair has to leave the skin is far shallower, and the result sits on someone’s face rather than under hair.
Eyebrows. A small number of grafts relative to a scalp case, but the least forgiving work in the field. Direction matters more than density — brow hairs change angle across the brow, and grafts placed at a uniform angle read as drawn on. Beard and moustache. Usually a larger graft count than brows and drawn from the same scalp donor area, which is the practical reason to plan a beard case alongside any scalp plan rather than separately. For either, send photographs first and get the plan and the figure in writing before booking anything. The same rule applies as for a scalp case: a quote that can move after you arrive is not a quote.

What does differ is the planning, and it differs enough that it is worth its own conversation. Female pattern loss is more often diffuse than a receding line, which means the donor area can be thinning too — and a donor area that is itself affected changes what is safely available to move. That assessment comes before any discussion of technique or tier.
The other practical difference is shaving. Unshaven and partially shaved approaches are more labour-intensive per graft than a fully shaved case, which is why they are not offered for every graft count. If not shaving is a firm condition for you, raise it at the first message rather than after a date is booked — it can change which approach is workable.
The reason it matters more than the price: a transplant redistributes hair, it does not create it. Where loss is diffuse and still progressing, moving grafts into a thinning area can produce a result that looks good at twelve months and disappointing later.
